"A Qantas Airbus A380 suffered an uncontained Rolls-Royce Trent 900 engine failure caused by a nonconforming thin-walled oil feed pipe."
The ATSB's investigation found that a manufacturing defect produced an abnormally thin pipe wall, leading to fatigue cracking, an oil fire and a destructive turbine-disc failure in November 2010. The 2013 investigation demonstrates how a small manufacturing defect can propagate into severe failures in a tightly coupled engineering system.
